Verification is not random and it is not personal. It is a set of rules, and most of them fire on the way out.

A withdrawal crossing an automated threshold
The most common trigger by a wide margin. Every operator sets an amount below which payouts process automatically and above which a human looks. That number is almost never published, it varies by operator, and it frequently varies by account.
The practical consequence is the one people get wrong: your first withdrawal being instant tells you where you were relative to the threshold, not that the threshold does not exist.
Balance far exceeding lifetime deposits

Payment method changes
Depositing by card and withdrawing to a wallet with no connection to it. Depositing from one chain and withdrawing to another. Neither is prohibited, both are noticed.

Source of funds review
